During your DOT physical, our provider will review your health history and complete a full evaluation that includes:

- Urinalysis (for medical screening, not drug testing)

- Overall physical examination
Pricing

- DOT Physical Exam: now $125 *limited time only (original price $175)

- DOT Retest (if required): $75
Please note that the $125 exam fee is non-refundable. If you do not pass your DOT physical, you may return for a retest at a discounted rate of $75 once the condition has been managed by your primary care provider and is under control.
We require a $25 deposit to secure your DOT physical appointment. This deposit is credited toward your final cost at the time of service. If you do not show up for your scheduled appointment, the deposit will be forfeited.
What to Bring
To make your appointment quick and efficient, please bring:

- A valid driver’s license or photo ID

- Your physical DOT medical form (please print and complete pages 1 & 2 upon arrival)

- Current Medical Examiner’s Certificate if renewing

- A list of any medications and dosages

- Glasses, contact lenses, or hearing aids if applicable

- Medical documentation for any existing health conditions such as diabetes, hypertension, or sleep apnea
